LCHAT was founded by the City of Liberty Parks and Recreation Department, Clay County Public Health Center, Liberty Hospital and Liberty Public Schools and comprise a governing board that oversees operations.
LCHAT’s vision is to create a community where healthy behaviors are the easy and preferred choice.
There are three key performance areas:
- Promoting Healthy Eating
- Increasing Physical Activity
- Advancing Social Inclusion
LCHAT’s goals and objectives for each performance area will shape public policies, practices and programs to improve the current health environment in Liberty, MO. These improvements are key to the community’s well-being; where you live matters to your health!
